Required Documents for the After-School Snack Program

Document	Requirement
Training Module	Before receiving snacks, all program staff members responsible for distributing snacks and completing all documentation must complete the appropriate training module.
Application	Prior to receiving snacks, one application per program must be submitted. Approval is given within 20 working days if the After School Program Questionnaire Review for each Program Official has been verified.
Questionnaire Review	All program staff members who will be distributing snacks must complete and submit the Questionnaire Review. This is an annual requirement.
Meal Count Form (Tally Sheet)	It is required for all programs that serve After School Snacks. Email FS After School Snack for access to your program's Meal Count Forms (Tally Sheets).
