Documents Required for Enrollment

A checklist of important documents and records to complete your child’s enrollment verification.

General (applies to grades TK-12, or as specified)

  1. Birth Certificate.  Documents verifying age, in order of desirability: birth certificate, baptismal certificate, passport, or Bible record.
  2. Proof of Residency:
    • Current utility bill (dated within the past 60 days) such as water, gas, electric, disposal service, rental agreement, or lease.

    • If living in the residence of another person, a Residency Affidavit from that person verifying the living situation must accompany the above verification at time of enrollment. The actual resident must also be present at the time of registration

  3. Special Education. A copy of your child’s IEP is required if your child receives special education services.
  4. Immunization Records/Shots (grades shown below). LEUSD requires evidence of completed required immunizations. Students with incomplete immunizations will not be enrolled (reference: SB 277).
    • Transitional Kindergarten
      • 4 POLIO (3 if last dose given after 4th birthday)
      • 5 DTP (4 if last dose given after 4th birthday)
      • 2 MMR (first dose must be given on or after 1st birthday)
      • 3 HEPATITIS B
      • 2 VARICELLA shots
    • Kindergarten
      • 4 POLIO ( (3 if last dose given after 4th birthday)
      • 5 DTP (4 if last dose given after 4th birthday)
      • 2 MMR (first dose must be given on or after 1st birthday)
      • 3 HEPATITIS B
      • 2 VARICELLA (chickenpox) shots
    • Grades 7 - 12
      • TDAP immunization record
  5. Physical Exam (Kindergarten, Grade 1). Must be dated after February 07, 2024.
  6. Oral Health Assessment (Kindergarten or Grade 1).
    • Must be dated after August 07, 2023.
    • To make sure your child is ready for school, California Law, Ed Code Section 49452.8 now requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school. Assessments that have been completed within the 12 months before your child enters school also meet this requirement.
    • The law specifies that the assessment must be done by a licensed dentist or other licensed or registered dental health professional.
    • District’s Oral Health Assessment forms must be used. This form is available for download at the right of this page.
